Certified Nursing Assistants & Geriatric Nursing Assistants

Duties:

  • Provides personal care and related services which includes: the ADL’ which is consistent with the State regulations, delegated and supervised by a registered nurse.
  • Tasks to be performed by a CNA/GNA/HHA must be assigned by and performed under the supervision of an RN who will be responsible for client services provided by the CNA/GNA/HHA.
  • Under no circumstances may a CNA/GNA/HHA be assigned to receive or reduce any intravenous procedures, or any other sterile or invasive procedures.

Responsibilities:

  1. Follows the plan of Care (POC) to provide safe and competent service to and for the client.
  2. Helps client to maintain good personal hygiene and assists in maintaining a healthful, clean and safe environment.
  3. Plans and prepares nutritious meals, runs errands when client is in need.
  4. Assists the client with ambulation and transfers ordered by a physician and approve by the nurse’s plan of care and when needed.
  5. Assists the therapy personnel as needed with rehabilitative processes.
  6. Encourages the client to become as independent as possible according to the plan of care.
  7. Attempts to promote the client’s mental alertness through involvement in activities of interest.
  8. Gives simple emotional and psychological support to the client and other members of the family which transmit trust and confidentiality.
  9. Reports any change in the client’s mental or physical condition or in the home situation to the staff nurse, or to the administrator.
  10. Performs routine light housekeeping tasks to ensure a safe and clean environment for the client.
  11. Documents clinical notes as well as ADL’s on a daily basis.
  12. Confirms on a weekly basis the scheduling of client visits so that other necessary visits by staff nurse can be coordinated.
  13. Works with personnel and other community agencies involved in the client’s service as directed by the nurse or the administrator.
  14. Attends MANDATORY in-service meetings as required by the regulation and agency policy.
  15. Notifies the Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings which will affect agreed service with the agency.

Job Conditions:

  1. The ability to travel, drive and the ability to access client’s home which may not be routinely wheelchair accessible are required.
  2. Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client’s condition to perform client care.
  3. On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and/or carry up to 30 pounds.
  4. Must be able to communicate clearly, both verbally in writing, in English.
  5. Must have reliable transportation.

Equipment Operation:

  1. Use of BP cuff, thermometer and stethoscope, proper hand washing materials.

Qualifications:

  1. Must have high school diploma & at least 2 years of experience.
  2. Must have up-dated CPR/First Aid certification.
  3. Ambulation and transfer of clients, including positioning
  4. Basic first aid and CPR training
  5. Caring for clients with special conditions and needs that are in the scope private duty
  6. Assistance with bathing, toileting, grooming, shaving, dental care, dressing and assistance with feeding
  7. Home management and medication management
  8. Home safety and sanitation
  9. Infection control in the home
  10. Medically related activities to include doctor visits and knowledge of how to take proper vital signs
  11. Proper nutritional information
  12. Must be free from health problems that may be injurious to client, co-workers and must present appropriate evidence to substantiate this
  13. Must comprehend the basis of personal care, housekeeping and meal preparation
  14. Must understand and respect clients including ethics and confidentiality
  15. Must have a cleared background screening
  16. Must have a current CPR certification
  17. Must have current physical and immunizations
  18. First Phase – Phone Interview with Personality Test & Skills, Knowledge & Competency Test (Pass with 85% – 1.5 hrs)
  19. Second Phase – Face to Face Interview
